The short answer
Inlays and onlays are custom porcelain restorations made in a lab and bonded into a tooth with a cavity or fracture too large for a filling but not large enough to need a full crown. An inlay fits within the cusps of a tooth; an onlay covers one or more cusps. They preserve more healthy tooth structure than crowns and can last 10–30 years.

Inlay, onlay or crown — how do we decide?
We always aim to save as much natural tooth as possible. If the cusps — the pointed chewing edges — are strong, an inlay is enough. If one or more cusps are weak or broken, an onlay rebuilds them. If most of the tooth is compromised, a crown is safest.

Inlays & Onlays FAQs
Are inlays and onlays better than fillings?
For large cavities, yes. They are stronger and more durable because they are crafted in a lab and bonded as a single piece.
Do onlays hurt?
The tooth is numbed during preparation. Most patients have mild sensitivity, if any, afterward.
Will an onlay look natural?
Yes. Porcelain is shade-matched and nearly invisible.
How long do onlays last?
With good care, 10 to 30 years.
